Explicit Consent Declaration Form of Data Owner

I accept, declare and undertake that I have read this Information Letter and, within the scope of the Personal Data Protection Law No. 6698, I have given my consent in advance to the collection, recording, processing and storage of personal data at the Refugees and Asylum Seekers Aid and Solidarity Association in accordance with the principles of the law.

In accordance with the Law on the Protection of Personal Data No. 6698 and the Regulation on the Processing and Confidentiality of Personal Health Data, the processing of my personal data, sensitive personal data, health data, including those that determine or serve to determine my identity, given verbally/written and/or electronically by me. All kinds of personal data, including the Refugees and Asylum Seekers Aid and Solidarity Association, to be processed by the Refugees and Asylum Seekers Aid and Solidarity Association, to choose candidates for the purpose of concluding an employment contract, to sign the employment contract and to provide health services in accordance with the employment contract and to plan these services, to other persons to whom sharing is required within the scope of the relevant legislation, and to be shared with organizations; I accept, declare and undertake that I consent to the processing of personal and special quality personal data, the details of which are given below, within the scope defined in the Personal Data Protection Law No. 6698 and the Regulation on the Processing and Privacy of Personal Health Data.

As the Human Resources Department of the Refugees and Asylum Seekers Aid and Solidarity Association, we attach importance to the legal protection and processing of Personal Data in accordance with the Law No. 6698 on the Protection of Personal Data, and we act  in all our planning and activities carefully . We do not only evaluate the protection and processing of Personal Data, which is the basis of the privacy of private life, in order to comply with the legislation, but also we include the value that we give to the human being to the the basis of our approach . With this awareness, as the Human Resources Department, we take all administrative and technical measures for the protection and processing of Personal Data. In order to ensure compliance with the policy, studies are carried out to raise the awareness of the employees, the necessary compliance processes are carried out for the new employees and the necessary arrangements are made.

Full Name, Home Address, Email Address, Citizenship Number (TCKN), Driver’s License Serial No, Identity Serial No, Face Scan, Date of Birth, Place of Birth, X-ray, MRI etc records, Blood group, Telephone Number, Country, City, Zip Code or District, Age, Gender, Criminal Record, Health records, Bank account number, CV, Academic information, Marriage status, Information about children, Emergency information, Disability status, Military records, etc.

What are the purposes and legal reasons for processing personal data?
Our institution is for both job candidates and working personnel;

  • Code of Obligations No. 6098,
  • Pursuant to the Personal Data Law No. 6698 and regulations,

The method of collecting personal data;
Your personal data in our institution; Filling in the job application form, CVs you shared, job applications made on our web page, filling in the forms defined in our procedures by our HR unit, on-the-job documents you have given in recruitment, forms you filled in trainings, AGI Forms, daily PDKS records are filled as a result of face scanning and requested information. they are saved through the HR Forms received.

How is personal data stored and protected?
Your personal data will be kept confidential in the database and systems of our institution in accordance with Article 12 of the KVKK; Except for legal obligations and the regulations specified in this document, it will not be shared with third parties in any way. Our institution is obliged to prevent the illegal processing of personal data in accordance with Article 12 of the KVKK, to prevent unauthorized access, to take software and physical security measures such as access management.

How long is the storage period of personal data?
Your Personal Data will be kept by us at all times as long as you work, for a period of 15 years in case you leave, considering the periods defined in the relevant laws.

What are your rights in accordance with Article 11 of KVKK?
Pursuant to KVKK, by applying to our institution, your personal data;
a) Learning whether it has been processed or not,
b) Requesting information if processed,
c) Learning the purpose of processing and whether it is used in accordance with its purpose,
d) Knowing the third parties to whom it has been transferred in the country/abroad,
e) Requesting correction if it is incomplete/wrongly processed,
f) Requesting anonymization and deletion within the framework of the conditions stipulated in Article 7 of the KVKK,
g) Requesting notification of the transactions made pursuant to subparagraphs (d) and (e) above, to the third parties to whom it has been transferred,
h) Objecting to the emergence of a result against you due to the analysis exclusively by automated systems,
i) You have the right to demand the compensation of the damage in case you suffer damage due to unlawful processing.
